Multiple Directors of Rehabilitation needed in Virginia (Near the Blue Ridge Mountains, near Norfolk, in Southern VA) and North Carolina (near Fayetteville).

Qualifications and Responsibilities

• 
  
• Valid and current license as a therapist or therapist assistant granted from the state of employment (PT/PTA/OT/COTA/SLP)
  
• Previous supervisory experience required
  
• Previous experience in Long-Term Care (LTC)/ Skilled Nursing (SNF) setting required
  
• Preferred knowledge of Patient Driven Payment Method (PDPM), but not required
  
• Participate in community planning, discharge planning, development and implementation of social care plans and resident assessments.
  
• Interview residents and families to obtain social history.
  
• Refer residents and families to appropriate social service agencies if needed.
  
• Provide information regarding Medicare/Medicaid, and other financial assistance programs available to the resident.
