    • Assess patients' understanding and provide education on the connection between diet, kidney disease, and treatment outcomes.
    • Educate patients/family/caregiver on nutritional status, fluid levels, and relevant lab parameters related to their condition.
    • Conduct thorough nutritional assessments and collaborate with the team to devise personalized care plans for patients.
    • Maintain accurate medical records, including progress notes.
    • Offer ongoing assessment and guidance to help patients maintain optimal nutritional health.
    • Identify malnourished individuals, coordinating interventions with the team.
    • Monitor and adjust nutrition therapy, documenting progress and addressing challenges.
    • Work with the team on Urea Kinetic Modeling recommendations.
    • Participate in nutrition Quality Assessment & Improvement (QAI) initiatives.
    • Review and interpret nutrition-related lab results.
    • Collaborate with physicians and facility staff as needed.
    • Maintain dietetic registration and fulfill continuing education requirements. EDUCATION:
      • Registered Dietitian accredited by the Commission on Dietetic Registration.
      • Board Certified Specialist in Renal Nutrition certification is a plus.
      • Valid state licensure if required. EXPERIENCE AND SKILLS:
        • Minimum 1 year of clinical nutrition experience as a Registered Dietitian.
        • Previous experience in renal nutrition is preferred. PHYSICAL DEMANDS AND WORKING CONDITIONS:
          • The job entails desk work, computer interaction, and engagement with patients, staff, and physicians. Travel between facilities and community locations is part of the role. Occasional travel to meetings may be needed.
          • Work environment mirrors a healthcare facility, with controlled temperatures and moderate noise levels. Potential exposure to infectious materials.
