  • Perform comprehensive nursing assessments and collaborate with interdisciplinary teams to develop individualized care plans
  • Set realistic, measurable short- and long-term goals with patients and families to reduce hospital readmissions
  • Administer medications and perform treatments in accordance with physician orders and facility protocols
  • Delegate appropriate care tasks to nursing assistants and supervise implementation of interventions
  • Maintain clear and timely communication across care teams and shiftsPosition Highlights:
