Overview: Support Patients. Honor Choices. Guide with Compassion. We are looking for a Remote MSW (Masters Social Worker) to cover our Pinehurst/Southern Pines, NC palliative care branch. This is a Remote position, but requires NC licensure and residency. Join a mission-driven team that is redefining how care is delivered to patients facing serious illness. As a Social Worker in our Advanced Illness Management (AIM) program, you will provide psychosocial support, guide patients and families through complex care decisions, and help navigate the emotional, spiritual, and social needs that accompany serious illness. You will work collaboratively with physicians, interdisciplinary teams (IDT), and community partners to deliver compassionate, patient-centered care aligned with individual values and goals. Schedule: Full Time Remote; Monday-Friday, 8am-5pm Licensure and Residency in North Carolina Required Essential Functions of Position Assess emotional, psychosocial, and mental health needs of patients and families facing serious illness Identify signs of distress, anticipatory grief, and coping challenges, offering targeted interventions Guide patients and families through goals-of-care conversations and the completion of Advance Directives or Do Not Resuscitate (DNR) orders Develop and maintain individualized care plans in collaboration with AIM, IDT, PCPs, and external providers Identify, coordinate, and advocate for access to community-based resources and support systems Respect and integrate cultural, spiritual, and family dynamics into care delivery Maintain thorough and timely documentation of visits, interventions, and care outcomes Adhere to HIPAA, Medicare regulations, company policies, and ethical social work standards Maintain updated lists of community resources and stay informed of changes in services or regulations Provide support through virtual visits from a secure home-based workspace About You: Education and Licensure Master’s degree in Social Work (MSW) Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Must hold or be able to obtain multistate licensure Experience Requirements Minimum 2 years of skilled social work experience Strong knowledge of health care systems and care options across multiple levels of care Experience in hospice, palliative care, geriatrics, or oncology preferred Demonstrated ability to deliver high-quality, compassionate support to patients and families Specialized Knowledge and Skills Skilled in crisis intervention, psychosocial assessment, and advance care planning Excellent communication, organization, and time management skills Collaborative and customer-service focused Competence in Microsoft Word, Excel, and Outlook Traits and Aptitudes Emotionally resilient and empathetic communicator Able to work independently while maintaining close collaboration with interdisciplinary teams Strong observational skills and attention to detail Committed to honoring the wishes and dignity of every patient Working Conditions Remote, work-from-home role supporting patients via telehealth Must maintain a secure and quiet work environment suitable for patient communications We Offer: Benefits for All Associates (Full-Time, Part-Time & Per Diem): Competitive Pay 401(k) with Company Match Career Advancement Opportunities National & Local Recognition Programs Teammate Assistance Fund Additional Full-Time Benefits: Medical, Dental, Vision Insurance Mileage Reimbursement or Fleet Vehicle Program Generous Paid Time Off 7 Paid Holidays Wellness Programs (Telemedicine, Diabetes Management, Joint & Spine Concierge Care) Education Support & Tuition Assistance (ASN to BSN, BSN to MSN) Free Continuing Education Units (CEUs) Company-paid Life & Long-Term Disability Insurance Voluntary Benefits (Pet, Critical Illness, Accident, LTC) Apply today to help us support patients, honor their choices, and guide families through some of life’s most important moments.
