Job Description:
Join Liberty Healthcare as a Home Care Registered Nurse and
contribute to a dedicated team providing innovative and tailored
healthcare services to our clients in their homes. This role
focuses on delivering high-quality, personalized care, aiming to
optimize the wellbeing of each client. You will implement care
plans, provide nursing services, and ensure a comfortable and
supportive environment for our clients. Assess, plan, implement and
evaluate nursing care plans in a home care environment. Monitor
patient health condition, record symptoms, and provide updates.
Administer medications, manage pain relief, and provide wound care.
Educate patients and their families on care strategies and disease
prevention. Coordinate with healthcare providers and
multidisciplinary team members. Ensure compliance with all safety
and health regulations. Strong clinical skills in patient care and
assessment. Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Proficiency in using healthcare documentation software. Medical
Insurance Active Registered Nurse (RN) license in the state of
employment. CPR certification required. Driver’s license and
reliable transportation.
