Position: Chronic Care Management Specialist Location: Raleigh, NC (on-site position) Schedule: Care Navigators can work Sundays - Saturdays from 8am to 9pm. We are looking for at least 36 hours per week of availability (Full Time and eligible for benefits). As a Chronic Care Management Specialist at Axil Health, you'll be the vital link between our patients and their healthcare team.
